Requirements
1. Be between the ages of 18 and 64. Child applications can be submitted by a parent or guardian.
2. Have an active email address.
3. Have a physical address in the State of Ohio.
4. The applicant must have reasonably good health and expect that they could be issued a policy (i.e. a person with diabetes, heart disease, of COPD may be declined).
5. Understand that, if APPROVED, a policy will be issued and an ID card will be sent to your address.
6. No premium payment is due with the application.
7. The policy will NOT go into effect until the first premium is paid, so there is no obligation on behalf of the applicant to keep the policy once applied for.
8. An Anthem Blue Cross and Blue Shield representative may call to verify applicant information. This may be necessary in order to approve an application.
9. Each application submitted and approved qualifies for the $25 Visa Gift Card, regardless of the number of people included on the application. For instance, a family of three filling out one online application would receive on Visa Gift Card for $25; however, the same family each submitting a separate application would receive three $25 Visa Gift Cards for a total of $75!
10. If the policy is kept for two full months (by paying the premiums), an additional $10 Visa Gift Card will be issued.
11. No Social Security Number is required.
12. An applicant cannot already have an Anthem Blue Cross and Blue Shield Health Insurance Policy.
